Hey, Im back. I have been gone for about 2 months and i have really changed. I really just want to go back on the server but i dont know how to prove i changed. I just REALLy have changed. I have been nice on every server since. I would just really hope you could bring me back and see that i have changed... PS: my dad just passed away....

Don't make excuses for your behavior, you did something wrong, take responsibility for it.
Make it easily readable and look like you actually cared enough to put time into it.
I can guarantee that you will have a much better chance if you are honest and show staff you've learned your lesson.
